Note Auditor & Clinical Compliance Guide

How to validate ABA session notes in seconds against CPT codes, payer documentation rules and clinical documentation standards.

No Patient Profile Required

Every review works without choosing a patient. Use client initials or an internal ID instead of names whenever possible; the plan check simply uses the BIP details you enter for that review.

What the History Keeps

Your Note Review history keeps the review result (date, CPT, score, status, findings and the client label you used). The note text itself is kept only while you share your reviews with a supervisor, and it is deleted if you revoke that consent.

The Chrome extension never stores note text in the browser. See HIPAA & Compliance for our safeguards.

The Note Auditor Engine

Note Auditor reviews ABA session notes before they are signed and billed. It catches documentation gaps, vague or subjective language, missing CPT 97153 / 97155 / 97156 / 97151 elements and notes that do not match the client’s plan, and shows exactly how to fix them.

Note Review

Run a review and keep the history of results: score, status and findings for every note.

New

Create a client plan (BIP): import it from a treatment plan document or fill it in step by step.

Patients

Optional client profiles with target behaviors and replacement programs used for plan-aware reviews.

Chrome Extension

The same review inside your web EHR, from the side panel, without copy-pasting.

Function 1: Start a Review

You can review a note from the web app or, more often, directly from your EHR with the extension.

  1. 1

    Step 1

    Open Note Review

    • In the web app, open Note Auditor → Note Review. In your EHR, open the AbaAiCheck side panel instead (see the extension workflow below).
  2. 2

    Step 2

    Set the context

    • Choose the CPT code (97153, 97155, 97156 or 97151) and, optionally, the session duration.
    • In the extension you also set Place of Service and Date of Service.
  3. 3

    Step 3

    Choose the plan source

    • Pick a patient to compare the note with their active plan, or choose No patient — use manual BIP details and type the target behaviors and plan goals for this review only.
    • To add a client plan for future reviews, use New: upload a treatment plan to import its data automatically, or complete it step by step.
  4. 4

    Step 4

    Run the review

    • Paste the session note and click Validate Note with AI, or Express QA & Smart Rewrite to get the review and a clean narrative in one step (depending on your plan).

Function 2: Note Review Deep Dive

What the AI Note Quality Reviewer returns for each note.

Score & Verdict

A 0–100 score with a clear verdict: Ready to bill, Needs fixes before signing or Do not bill until the note is corrected.

CPT Guideline Checklist

Which guidelines for the selected CPT code are documented (for example, interventions delivered, data collected, caregiver participation) and which are missing.

ABC Analysis

Antecedent, behavior and consequence detected in the note, and whether target behaviors and replacement programs match the client’s plan.

Objective Language & Rewrites

Subjective or emotional phrasing (for example, “client was bad today”) is flagged with objective, measurable alternatives. Missing facts appear as [bracketed] placeholders, never invented.

Session Time

When you enter the session duration, time-related documentation is checked against the CPT code; the Billing & Claims Auditor then checks billed units against time.

Smart Rewrite

A compliant rewrite of the narrative that keeps only what the note documents, ready for you to review and paste.

Function 3: Patients & BIP Context

Client profiles are optional and exist only to make reviews plan-aware.

  • A profile stores the client label you choose, target behaviors, replacement programs and goals from the BIP or service plan.
  • With a profile selected, the review checks whether the note addresses the plan’s behaviors and programs; without one, it uses the manual BIP details you type, or only the CPT rules.
  • On OfficePuzzle, the extension can also read the active service plan for the client of the note you have open.
  • Minimal-PHI option: run every review without a profile and with client initials or IDs only.

In-EHR Extension Workflow

Run Note Auditor next to the note you already have open.

  1. 1

    Step 1

Open the note in your EHR

    • Open the session note in your web-based EHR, then click the AbaAiCheck icon to open the side panel.
  2. 2

    Step 2

    Pick the engine and context

    • Choose an engine in the AI Engine menu (for example, Express QA & Rewrite or Smart Rewrite Assist), then the CPT code, Place of Service and date.
  3. 3

    Step 3

    Review in one click

    • Click the run button (for example, Run Express QA). One-click capture is built and tested with OfficePuzzle; on other web EHRs such as CentralReach, select or paste the note text into the side panel.
    • Read the score and findings, apply the rewrite you approve, and sign the note in your EHR.

The clinician reviews every suggestion and remains responsible for the signed note.

Review your next note before you sign it

Note Auditor is included in every AbaAiCheck plan, with a free trial to start.
