Compares both
Every listed or described behavior and program is matched against the active goals — old, inactive plans are ignored.
What gets flagged
Out-of-plan behaviors or skills
Documented without being an active goal and without a justification as an emerging behavior (with monitoring and medical necessity). At most one or two emerging behaviors are accepted per note.
Unaddressed goals
Plan behaviors and programs that the note does not address, shown as covered / missing so the clinician can complete the narrative.
Missing narrative data
Behaviors listed in the header but missing antecedent, intervention or client response in the Summary.
Preventing billing rejections from out-of-scope documentation
Payers pay for the treatment they authorized. When a session note cannot be tied back to the active plan, it invites questions about medical necessity. The cross-check shows the clinician exactly which items to justify or correct — while the note is still a draft.
