Why hallucinated clinical facts are dangerous in a Medicaid audit
A session note is a legal record that supports a claim. If an AI tool adds a prompt level, a trial count or a reinforcer the clinician never documented, and the note is signed, the record no longer reflects the service that was delivered. In an audit, documentation that does not support the billed service can lead to recoupment (clawbacks) — and invented details can look like falsified documentation.

How the Assertive Bracket System works
The engine drafts the note in ABC format using only the facts the note documents. It is instructed never to invent a missing variable: instead it inserts a standardized bracket where the fact belongs. The draft cannot be re-checked while brackets remain, and the verified score comes from auditing the completed note.

What "zero-hallucination" means here
It is a design rule, not a promise that AI can never be wrong: the model is instructed never to state a fact the note does not contain, the Date of Service is protected by a code-level check, and nothing is final until a human completes the brackets and re-checks the note. That combination is what keeps an ABA session note audit defensible.
