The Danger of Using Generic AI (ChatGPT) for ABA Session Notes
Pasting a session note into a consumer chatbot and asking it to "make it sound professional" is fast. It can also expose protected health information and put invented clinical details into a signed Medicaid record.
HIPAA compliance risk
Session notes contain protected health information (PHI). Under HIPAA, a covered entity may share PHI with a vendor only under a Business Associate Agreement (BAA). Consumer versions of general-purpose chatbots are generally not covered by a BAA, so pasting a client's note into one can be an impermissible disclosure — even if the name is removed, notes are full of identifying details.

Medicaid audit risk: hallucinated clinical details
Large language models are designed to produce plausible text. Asked to "complete" a thin note, they will happily add a prompt level, a number of trials or a reinforcer. Once the clinician signs, those invented details become part of the medical record. If an auditor compares the note with data sheets or the service that was actually delivered, documentation that does not reflect the service can lead to recoupment — and fabricated details can raise questions of false documentation.

The numbers, the prompt level and the reinforcement duration were never in the note — a generic model invented them.
